Description: This permit is being extended on 02-26-2009 and will end on 03-15-2009. by accepting this permit permittee acknowledges the following: a) that time is of the essence; b) they have satisfied themselves that the owner has provided satisfactory r/w and/or easement(s) in accordance with city code to accommodate the work; c) to give city & adjacent residents 48 hrs advance notice prior to starting work; d) to construct the improvements as indicated on the permit plan record documents under plan file no. 2008-168b and contract no. 1 concerning agnes and bellfontaine avenues with work not starting prior to 11/24/08; (city agrees to early issuance of this permit subject to applicant agreeing to pay a triple permit fee if work is started before the start date indicated herein); e) to complete this work by the permit completion date of 11/30/08; f) to coordinate with the work of all other contractors for timely completion; g) to remove/relocate/reconstruct unacceptable work to the satisfaction of the city; h) to remain responsible for the work until permit and bond are released; i) the city will notify the permittee by letter when the maintenance period starts if applicable under the provisions of the bond for this permit. maintenance shall include any and all repairs to damaged components of the facility that may have occurred during the maintenance period; j) the city will notify the permittee by letter when this permit and the facility constructed under it are final accepted. final acceptance shall not occur until the maintenance period required under the bond has been completed and all maintenance items addressed. k) they have satisfied themselves that subgrade conditions are satisfactory and suitable for installation of the improvements proposed under the scope of this permit.
